P. acnes is a Gram-positive pleomorphic diphtheroid-like anaerobic bacillus, and it is part of the normal skin flora. 2, 5 As identified previously, shunt infection with P. acnes frequently results in a mild clinical picture, can be associated with an abdominal pseudocyst and infections occur in the older child and adolescent. 4 With improved diagnostics, including extended culture protocols, P. acnes has been specifically recognized as a dominant organism in infections involving shoulder prostheses [ 14–23 ]. Yet, there has been a paucity of data on the clinical and microbiologic characteristics of such infections. Both early and delayed infection were observed in our patient population, which suggests that P. acnes may remain latent for several months before an infection is diagnosed. Early infection was associated with local joint inflammation more frequently than delayed infection.
